The Northumberland Family Health Team is governed by a doctor-led Board of Directors. Reporting through the Executive Director to the Board is a clinical team and an administrative team who work in tandem to attend to all the clinical, program and administrative matters on behalf of the organization. As additional members are recruited we will update THE TEAM accordingly.

The goal of the Diabetes program is to help the NFHT patients living with diabetes maintain and improve their health. Self-management education is provided through individual appointments and group workshops. Services are offered in collaboration with the Port Hope Community Health Centre Diabetes Team in their Port Hope, Cobourg and Colborne sites. Adult Mental Health Program.
