To improve the quality of life for persons diagnosed with a serious and persistent mental illness by providing social, recreational, and educational opportunities in a safe environment. The County Connection Clubhouse is a drop in/recreational center that serves persons living in Sibley County who are diagnosed with a serious and persistent mental illness. 2013 South Central Community Based Initiative. The Clubhouse is a resource for consumers who are diagnosed with a serious and persistent mental illness. Talk to your Case Manager about becoming a member. Your Case Manager can provide you with the necessary application to become a clubhouse member. Eligibility requirements for the County Connection Clubhouse are as follows:. 18 years of age or older. Diagnosed with a serious and persistent mental illness. Receiving Adult Mental Health Case Management. 2013 South Central Community Based Initiative.
